POST-OFFER MEDICAL QUESTIONNAIRE

 

Policy

 

 

The Board of Education requires every applicant given a conditional offer of employment in the job categories of custodial staff, food service staff, school nurse, special education instructional assistant, coach and any other health sensitive positions to complete a Medical History Questionnaire no later than seven business days after the receipt of the conditional offer of employment.  The Medical History Questionnaire shall be supplied by the Superintendent and/or his or her designee.  The purpose of the Medical History Questionnaire is to determine whether the applicant currently has the ability to perform, with or without reasonable accommodation, the essential functions of the position without posing a direct threat to the health and safety of the individual or others.

 

Every applicant who receives a conditional offer of employment in the aforementioned job categories must complete and return the Medical History Questionnaire to the Central Office Personnel Department within the timeframe set above from the conditional offer of employment.  The Board of Education shall maintain the completed Medical History Questionnaire in a separate, confidential medical file in accordance with applicable law.
