Physical Assessments

Policy

It is the policy of the Region 15 Board of Education that physical assessments, to be recorded on the State of Connecticut Health Assessment Record Forms, shall be completed for those students entering school and in grades kindergarten, six, and ten.

New students to Region 15 must present a copy of the State health form from their previous school district.  This applies to Connecticut residents only, assuming the student has previously attended a public school.  New students to Region 15 moving in from outside of Connecticut or from a private school must present the written results of a physical examination that is thirteen months old or less on the day of entrance and proof of State required immunizations.  Failure to meet these requirements will cause the student to be excluded from school until such time as the assessment and immunizations have been completed.

Preschool or Kindergarten physical assessment reports shall be filed with the appropriate school nurse on the day of registration.  All other physical assessments due shall be filed with the appropriate school nurse by February 15.  Failure to meet this requirement will cause the student to be excluded from school until such time as the assessment has been submitted.

All physical assessments, including sports physicals, shall be completed by a legally qualified practitioner of medicine, at the expense of the student/parent.  If for financial or other extraordinary reasons, such service is not available to students, the physical assessments shall be conducted by the school medical advisor or designee.
