transfer student registration

 

Policy

 

 

 

It is the policy of the Region 15 Board of Education that any student transferring into Regional District 15 schools must be resident of the Region, must be enrolled by the parent or guardian (if under 18 years of age), and must present to the school Principal an official transfer or other written records from the previous school.

1.

Any transfer student (under the age of 18 years) enrolling in any school in Regional District 15 must be accompanied by a parent or guardian.

2.

If an official transfer is not available, the parent will present the child’s birth certificate, last report card, or other written evidence which indicates the child’s grade placement and attendance record.

3.

If no documentation is available the Principal will:

 

a).

Enroll the child in school;

 

b.)

Tentatively place him/her at what appears to be an appropriate grade level with the understanding that if another grade level is indicated when the records are received the child’s grade placement will be adjusted accordingly; and

 

c.

Have the parents sign a release of information form and send for the child’s records immediately.

4.

Each transfer student shall submit health records of physical assessments described in Region 15 Board of Education Policy No. 5141, Physical Assessment.

5.

Each transfer student shall submit the following proof of residency:

 

1.

Mortgage or rental agreement showing the family’s name at the Middlebury or Southbury address.

 

2.

If the family cannot produce a mortgage or rental agreement, then we will accept a driver’s license, car registration, and a copy of a personal property tax for the car from the Town of Middlebury or Southbury.  These items must show the family’s name at the Middlebury or Southbury address.  One or both of the attached affidavits are also required.

 

3.

If none of the above can be provided, then a utility bill with a canceled check is next in line.  The utility bill must show the family’s name at the Middlebury or Southbury address.

 

 

 

 

 

If the family has any special circumstances, the school Principal or the Superintendent will meet with you to discuss alternative processes for proving residency.
