Life Insurance

Group Life Insurance coverage is available according to the terms of the employee group contract. Major life changes may prompt the need to change your life insurance beneficiary. If you need to change your beneficiary, please do so by clicking the link below. Return the signed original form to the Region 15 Human Resources department at Central Office.
