Immunizations
New students must submit a copy of their immunizations to the school nurse, prior to entering school.
PRESCHOOL (Children entering after age 3 but before age 5)
Diphtheria, Tetanus, Pertussis (DTaP) — 4 doses
Polio (IPV/OPV) — 3 doses
Measles, Mumps, Rubella (MMR) — 1 dose on or after the first birthday
Hepatitis B — 3 doses, last one on or after 24 weeks of age
Varicella (Chicken Pox) — 1 dose required on or after the first birthday, or verification of the disease.
Hib — 1 dose must be given on or after the first birthday
Pneumococcal - 1 dose on or after the first birthday
Influenza - 1 dose administered each year between August 1 - December 31st. (2 doses separated by at least 28 days required for those receiving flu for the first time.)
Hepatitis A - 2 doses given six months apart, 1st dose on or after first birthday.
KINDERGARTEN
Diphtheria, Tetanus, Pertussis (DTaP) — At least 4 doses. The last dose must be given on or after 4th birthday.
Polio (IPV/OPV) — At least 3 doses. The last dose must be given on or after 4th birthday.
Measles, Mumps, Rubella (MMR) — 2 doses separated by at least 28 days. First dose on or after the 1st birthday.
Hepatitis B — 3 doses, last dose on or after 24 weeks of age
Varicella (Chicken Pox) — 2 doses separated by at least 3 months; First dose on or after the first birthday, or verification of the disease.
Hib — First dose on or after first birthday for children less than 5 years old.
Pneumococcal - First dose on or after the 1st birthday for children less than 5 years old.
Hepatitis A - Two doses given six calendar months apart, 1st dose on or after 1st birthday.
GRADE 1
Diphtheria, Tetanus, Pertussis (DTaP)/Td/Tdap — At least 4 doses. The last dose must be given on or after 4th birthday.
Polio (IPV/OPV) — At least 3 doses. The last dose must be given on or after 4th birthday.
Measles, Mumps, Rubella (MMR) — 2 doses separated by at least 28 days. First dose on or after the 1st birthday.
Hepatitis B — 3 doses, last dose on or after 24 weeks of age.

GRADES 2 - 6

Polio (IPV/OPV) — At least 3 doses. The last dose must be given on or after 4th birthday.

GRADE 7 - 8
Td/Tdap — 1 dose for students who have completed their primary DTaP series. Students who start the series at age 7 or older only need a total of 3 doses of tetanus-diphtheria containing vaccine, one of which must be Tdap.
Polio (IPV/OPV) — At least 3 doses. The last dose must be given on or after 4th birthday.
Measles, Mumps, Rubella (MMR) — 2 doses separated by at least 28 days. First dose on or after the 1st birthday.
Meningococcal - 1 dose
Hepatitis B — 3 doses, last dose on or after 24 weeks of age
Varicella (Chicken Pox) — 2 doses separated by at least 3 months. First dose on or after the 1st birthday, or verification of the disease.
GRADE 9-12
Td/Tdap — At least 3 doses. The last dose must be given on or after 4th birthday. Students who start the series at age 7 or older only need a total of 3 doses of tetanus-diphtheria containing vaccine, one of which must be Tdap.
Polio (IPV/OPV) — At least 3 doses. The last dose must be given on or after 4th birthday.
Measles, Mumps, Rubella (MMR) — 2 doses separated by at least 28 days. First dose on or after the 1st birthday.
Hepatitis B — 3 doses, last dose on or after 24 weeks of age
Varicella (Chicken Pox) — For students <13 years of age, 1 dose given on or after first birthday; for unvaccinated students 13 years of age or older, 2 doses given at least 4 weeks apart; or verification of the disease.
Important Reminders
- DTaP vaccine is not given on or after 7th birthday.
- DTaP may be given for all doses in the primary series.
- Tdap can be given in lieu of Td vaccine for children 7 years and older unless contraindicated. Tdap is only licensed for one dose.
- Hib is not given or required for children 5 years of age or older.
- Pneumococcal is not required for children 5 years of age or older.
- Hepatitis A is required for all Pre-K and K students born on or after 1/1/2007 and less than 5 years of age. The vaccine will be required for all Kindergarten students born on or after 1/1/2007 in the 2012-13 school year.
- Hep B requirement for school year 2012-13 applies to all students in grades K-12. Spacing intervals for a valid Hep B series are at least 4 weeks between dose #1 and dose #2; 8 weeks between dose #2 and dose #3; at least 16 weeks between dose #1 and dose #3; dose #3 should not be given before 24 weeks of age.
- Second MMR for school year 2012-13 applies to all students in grades K-12.
- Laboratory confirmation of immunity is only acceptable for Hepatitis B, Measles, Mumps, Rubella, Hepatitis A, and Varicella.
- VERIFICATION OF VARICELLA DISEASE: Confirmation in writing by a MD, PA, or APRN that the child has a previous history of disease, based on family or medical history.

Physical Examinations
New students must submit a complete physical to the school nurse prior to entering school.
A. This physical must be on the State of Connecticut Health Assessment Record (HAR) form.
B. All * boxes must be completed by the physician including: height, weight, blood pressure, HCT/HGB, gross dental, scoliosis, Tuberculin test (for students coming from another country), vision, and auditory evaluations prior to admission to school. (Please note that vision and auditory screenings will not be done by the school on admission — these should be performed as part of the physical examination.)
C. New entrants to Region 15 must present the written results of a physical examination that is thirteen (13) months old or less on the day of entrance and proof of State required immunizations. ("New Entrants": Students new to the school district, students entering pre-Kindergarten, entering Kindergarten, moving in from outside of Connecticut or another country, or transferring from a charter, parochial, or private school in Connecticut. One exception is students returning from private approved special education placements - they are not considered new entrants).
D. Parent/guardian should complete and sign the front page of the Health Assessment Record (HAR).
Medications
All medications (including over-the-counter drugs) may only be administered by the school nurse with a written physician’s order and written permission of the parent or guardian. Such medications must be delivered to the school nurse by the parent/guardian in the original container or prescription bottle. Self-administered medication (such as asthma inhalers) may only be used with a physician’s written order and written permission from the parent or guardian.
