Welcome to Pomperaug Regional School District 15, serving the communities of Middlebury and Southbury, Connecticut. The directions for registering your incoming Kindergartner in Region 15 schools are outlined below.

Kindergarten Orientation Meeting for Principal and Parents:
Parents/Guardians are invited to attend an informational meeting with the school principal at your child's school on March 29, 2012 at 7:00 p.m. to go over the kindergarten curriculum and to introduce the kindergarten teachers. At this meeting, parents/guardians can sign up for their Registration Date/Time. If you are unable to attend this meeting and would like to schedule a Registration Date, please contact the school directly.

The following forms must be printed and completed by the child's parent or guardian prior to Registration Day, as applicable. Please bring your child's birth certificate to registration.

• Health & Development Questionnaire
• Health Assessment Record from the State of CT Dept. of Education 
  NOTES: 
  -   The child's physical examination must be no older than 13 months old; dated on or after July 31, 2011. See Immunizations for more information.
  -   New state requirements may make it necessary to revise/update your child’s physical at a later date.

• Pre-School Parent/Guardian Permission Form  Upon the parent/guardian's permission, Region 15 will send a letter and questionnaire to your child's pre-school or child care center director to assist in class placement decisions; to help us anticipate any support children may need; and to assist us with instructional planning.
