Welcome to the 2011-12 school year. The goal of the School Health Program of Region 15 is to promote, protect, maintain, and improve student health. The school nurse is a health advocate for all students and coordinates the health services of school, home and community. The school nurse implements state laws, local board policies and administrative procedures relative to health services in assigned schools.

Please refer to the Student Health information as well as the following information below. 

Absences

A parent or guardian needs to call the School Nurse at 203-264-6811 before 9:00 a.m. (a message can be left at anytime during the night or early morning). Please be specific with the symptoms your child is experiencing. Remember, absences for travel are unexcused unless the Principal gives a special exception in advance.

If your child arrives to school after 9:00 a.m., they must sign in at the GES main office and will be marked tardy. 

It is important for children to attend school regularly. However, if your child is ill (ie. fever, vomiting/diarrhea) we ask that your child be fever/illness free for 24 hours prior to returning to school.  Please try to avoid scheduling routine medical, dental, and orthodonture appointments during the school day.

Health Records

A cumulative Health Record for each student will be maintained in a secure location in the Nurse’s Office. Access to these records is limited to the school nurse. The nurse will relay pertinent information to appropriate school personnel.

Emergency Forms

Students will be given emergency forms at the beginning of each school year. It is important that this form be completely filled out and returned promptly. Three adult emergency contacts should be listed other than the parents. These contacts should be easily accessible in the parents’ absence. Babysitters, daycare providers, and neighbors should also be listed.

If any change in the information occurs during the school year, please notify the school nurse immediately. 

Emergency Notification and Transportation

When any severe accident or acute illness occurs, a parent or person listed on the student’s emergency card will be notified. Transportation for the student who must leave school will be provided by the parent/guardian except when the nurse determines that the student must be taken to a hospital by ambulance.

Recess and Physical Education

Children are expected to come to school properly clothed for weather conditions. Hat, coat, gloves, snow pants, and boots are required to play in the snow. For an excused absence from Physical Education for more than two consecutive periods during the school year, a doctor’s excuse, in writing is necessary. If the doctor’s written excuse states “until further notice,” a second written note is required to return to Physical Education.

Kindergarten Registration

All forms for Kindergarten Registration can be found here on the Region 15 website.  The Health Assessment Form for incoming kindergartners (completed by the physician) must have all “starred areas” completed and sent to the school as soon as possible.  The exam must have done after July 31st for the upcoming school year.

Physicals for Upcoming 6th Graders

To all 5th Grade Parents:  If your child is entering 6th grade in Fall 2011, (s)he is required to have a physical on the State of Connecticut form. The physical MUST be dated on or after July 1, 2011 for it to be considered their 6th Grade Physical. All physicals are valid for 13 months. Please return the completed form to the appropriate middle school nurse after July 1, 2011.
