Memorial Middle School

The State of Connecticut has implemented new immunization requirements for students entering seventh grade. The new requirements will coincide with the national recommendations. The new requirements are effective August 1, 2011. Any student entering seventh grade for the 2011-2012 year MUST have the following proof of immunizations:

** Second Varicella Vaccine (or proof of disease- month & year)

** Tdap

* Meningococcal Vaccine

Verification of the above must be signed by an MD, DO, APRN or PA

(PA signature must be accompanied by MD stamp or signature)
