Please remember to call your student absent or tardy to Attendance Line at 203-758-1912 before 8:00 a.m.  Students must be home for at least 24 hours after fever, diarrhea or vomiting symptoms cease. Make sure students are well enough to participate in a full day of school.

Gym Excuses: Please give all doctor's excuses for injuries and restrictions to the Nurse.

Immunizations: Check the new State of CT Immunization requirements.

 NEW!  Physicals for Upcoming 6th Graders

To all 5th Grade Parents:  If your child is entering 6th grade in Fall 2011, (s)he is required to have a physical on the State of Connecticut form. The physical MUST be dated on or after July 1, 2011 for it to be considered their 6th Grade Physical. All physicals are valid for 13 months. Please return the completed form to the appropriate middle school nurse after July 1, 2011. 
 

Physicals are needed yearly if your student is trying out for a sport (they are good for 13 months from the date of the physical. - See Link Above)  Note: No student may tryout without a valid physical.

Field TripsAny student in need of a medication on a field trip MUST notify the nurse’s office one (1) week prior to the field trip. A doctor’s order is mandatory for prescription or over the counter medications. Also notify the nurse’s office of any medical conditions that students may have (even if we are already aware of it.)  NOTE: Parents are responsible to bring in the child's medication for field trips from a home supply. Medications from school WILL NOT be used.

Tuberculosis: To be in compliance with State of Connecticut Regulations, Region 15 adopted a Tuberculosis policy in February 2006. Students entering Region 15 from outside of the United States must have a TB screening with results interpreted/documented by their physician's office prior to entering the school district. Additional screening may be required according to the Tuberculosis policy. Please contact  the  Health Office with any questions.

1.) Students entering the Region from outside of the U.S. must have a TB screening with the results interpreted and documented by their physician's office PRIOR to entering Region 15.

2.) Additional TB screening may be required by the school medical advisor and/or school nurse under the following circumstances:

a. if a student has traveled to a high risk country, stayed for at least a week with substantial contact with a high risk population,
b. if a student has had substantial contact with someone from a high risk country,
c. if a student has had contact with someone suspected to have TB,
d. if a student has had contact with anyone who has been in a homeless shelter, jail, prison, uses illegal drugs, or has HIV infection.

If you will be traveling on vacation to any of the areas listed below, this policy must be adhered to. Please contact the Nurse's Office with any questions. This may require 2-3 additional absent days for your child.
(High risk countries noted by state: All countries in Africa, Asia, <including former Soviet Union>, Eastern Europe, Central and South America, Dominican Republic and Haiti.)

Allergy Alert: MMS has a number of students with peanut allergies. Please keep this in mind when sending in snacks that may contain peanuts. We would like to minimize any potential exposures that may negatively affect students.
