Pomperaug Elementary School

The State of Connecticut has specified certain immunizations and screenings that must be complete in order to attend school. These items are noted with an asterisk on the blue physical form sent home to you. Please call the nurse at 203-264-6230 if you have any questions regarding the required immunizations or screenings.

Physical examinations are required for entrance into preschool or kindergarten. Any new student entering PES must show written proof of a physical exam that is less than 13 months old.
