Region 15 Schools are not in session on Monday, September 25, 2023 for Yom Kippur.
Classes resume on Tuesday, September 26, 2023.

Our Kindergarten enrollment period begins in January for the upcoming school year. To attend kindergarten in the 2023-24 school year, your child must be 5 years of age before January 1, 2024.

Prior to registering a child in Region 15, we require proof of residency. Permanent residency (physical presence with an intention to remain) in Southbury and/or Middlebury is the primary requirement to enroll in Region 15 schools. A landowner, taxpayer, or business owner does not confer residency privileges for school enrollment processes.
Registration cannot be completed until all documents have been received and confirmation of Connecticut State Health compliance has been received from the school nurse.
The following documentation can be sent by email or brought in person by appointment to the Registrar at 286 Whittemore Rd. Middlebury, CT, 06762.
-AND-
Provide one of the following secondary documents in the name of the Parent/Guardian(s) with matching addresses:
Current Utility Bill- landline phone, water, electric, gas, oil
If the parent/guardian is unable to provide proof of residency due to living with a family member or friend (Host resident):
A Host’s Statement Residency Affidavit must be completed and notarized with the signature of the family member or friend (Host resident).
Proof of residency documents listed above must be provided by the Host resident.
If the child does not reside with his/her parents/guardians but does live in Middlebury or Southbury with a family member or friend (Host resident):
A Student Residency Affidavit, Parent Permission Statement and Host Statement must be completed and notarized with the signature of the parent/guardians and host.
Proof of residency documents listed above must be provided by the Host resident.
Alternative methods for providing residency may be permitted. Please discuss your situation with the Registrar.
The Federal McKinney-Vento Homeless Assistance Act provides special legal protections for children and youth experiencing homelessness, including homeless “unaccompanied youth” who are not in the custody of a parent or guardian. Children experiencing homelessness or displacement have a right to enroll in school even if their families cannot produce enrollment documentation. Assistance is available at the time of registration.

Connecticut Health Assessment Record Must have had physical after 7/30/2022 and have it completed on this form, we cannot accept Early Childhood Health Assessments for Kindergarten Registration.
We can not accept photographs of State Health Assessment Records.
School nurse and other health information including required immunizations can be found on the Region 15 Health Services page.
Copy of most recent school transcripts, grades, test scores and credits.
