Kindergarten Registration

We are excited for your child to begin their educational journey in Region 15!

Our Kindergarten enrollment period begins in January for the upcoming school year. To attend kindergarten in the 2023-24 school year, your child must be 5 years of age before January 1, 2024.

2023-24 Kindergarten Information:

Please complete the KINDERGARTEN INITIAL REGISTRATION FORM to begin the Kindergarten Registration process.

After completing the initial registration form, you will be contacted by your child's school.  In the meantime, you can begin to collect the documents listed below.

Important Dates:

  • Thursday, March 2, 2023, at 6:00 p.m. for PES, MES, and GES and at 5:30 pm for LMES (Snow date 3/9/23) Parent/Guardian Information Session at each elementary school 
  • Tuesday, May 30th, to Friday, June 2nd, 2023: Individual Registration Appointments at each elementary school.  Elementary schools will contact families directly for appointments.

Enrollment and Residency Requirements

Prior to registering a child in Region 15, we require proof of residency.  Permanent residency (physical presence with an intention to remain) in Southbury and/or Middlebury is the primary requirement to enroll in Region 15 schools.  A landowner, taxpayer, or business owner does not confer residency privileges for school enrollment processes.   

Registration cannot be completed until all documents have been received and confirmation of Connecticut State Health compliance has been received from the school nurse.

The following documentation can be sent by email or brought in person by appointment to the Registrar at 286 Whittemore Rd. Middlebury, CT, 06762.

Registration Forms

PROOF OF RESIDENCY

Provide one of the following primary documents in the name of the Parent/Guardian(s): 
  • Current Mortgage Statement
  • Lease Agreement
  • Deed
  • Property Tax Bill

-AND-

Provide one of the following secondary documents in the name of the Parent/Guardian(s) with matching addresses:

  • Current Utility Bill- landline phone, water, electric, gas, oil

If the parent/guardian is unable to provide proof of residency due to living with a family member or friend (Host resident):

  • A Host’s Statement Residency Affidavit must be completed and notarized with the signature of the family member or friend (Host resident).  

  • Proof of residency documents listed above must be provided by the Host resident.

If the child does not reside with his/her parents/guardians but does live in Middlebury or Southbury with a family member or friend (Host resident):

Alternative methods for providing residency may be permitted.  Please discuss your situation with the Registrar.

The Federal McKinney-Vento Homeless Assistance Act provides special legal protections for children and youth experiencing homelessness, including homeless “unaccompanied youth” who are not in the custody of a parent or guardian. Children experiencing homelessness or displacement have a right to enroll in school even if their families cannot produce enrollment documentation. Assistance is available at the time of registration. 

PROOF OF GUARDIANSHIP

Parent(s) of Student:

  • Photo ID: Driver’s License or State Issued Identification or Unexpired Passport

Legal Guardian(s): 

  • Court Document indicating custody or guardianship

  • Photo ID: Driver’s License or State Issued Identification or Unexpired Passport

In the case of divorce, please provide a copy of the Custody Agreement at Registration.

PROOF OF AGE

  • Child’s Birth Certificate or Unexpired Passport

Alternative methods for providing proof of age may be permitted.  Please discuss your situation with the Registrar.

STUDENT HEALTH RECORDS

  • Connecticut Health Assessment Record Must have had physical after 7/30/2022 and have it completed on this form, we cannot accept Early Childhood Health Assessments for Kindergarten Registration.

We can not accept photographs of State Health Assessment Records.

School nurse and other health information including required immunizations can be found on the Region 15 Health Services page.
