Kindergarten Registration
We are excited for your child to begin their educational journey in Region 15!
Our Kindergarten enrollment period is now open for the upcoming 2026-27 school year.
Public Act 23-208 requires students to be 5 years old by September 1, 2026 to enroll in Kindergarten for the 2026-27 school year. Region 15 will accept students who will turn age 5 by January 1, 2027 per written request from the family.
For students born between September 2, 2021 and January 1, 2022, you will be contacted by email with more information regarding a screening assessment in the areas of academic, social, physical, health, behavioral, and communication skills after completion of the Kindergarten Initial Registration Form.

To make the process go smoothly please review the required documentation listed below. All documents must be uploaded during the registration process. Please note you can save your registration and return to the process.
Proof of Residency (required): One document from Category A and one document from Category B. Alternative methods for providing residency may be permitted. Please email Registration@Region15.org.
- Category A: Current Mortgage Statement, Lease Agreement, or Property Tax Bill.
- Category B: Current Utility Bill (landline phone, water, electric, gas, or oil).
- Parent/Guardian Residency Affidavit
For students residing with parents/guardians who are unable to provide a Primary Proof of Residence (Mortgage Statement, Deed, Rental Agreement) - Host Statement of Residency - Affidavit
For families living with someone who is the owner/tenant of a residence in Middlebury or Southbury, CT - Student Residency Affidavit, Parent Permission Statement and Host Statement
For students residing with parents/guardians and living with someone who is the owner/tenant of a residence -
Alternative methods for providing residency may be permitted. Please discuss your situation with the Registrar.
The Federal McKinney-Vento Homeless Assistance Act provides special legal protections for children and youth experiencing homelessness, including homeless “unaccompanied youth” who are not in the custody of a parent or guardian. Children experiencing homelessness or displacement have a right to enroll in school even if their families cannot produce enrollment documentation. Assistance is available at the time of registration.
Parent/Guardian Information:
- Photo Identification: Driver’s License, State Issued ID, or Unexpired Passport.
- Custody Agreement: (if applicable).
Proof of Age: Alternative methods for providing proof of age may be permitted. Please email Registration@Region15.org.
- Child’s Birth Certificate or Unexpired Passport.
Student Health Records:
- State of Connecticut Health Assessment Record Connecticut Health Assessment Record (Photographs of assessment records can not be accepted, only scanned or originals).

- All incoming students must have had an exam within the last 12 months.

A Parent/Guardian Kindergarten Information Night will take place at each elementary school on Thursday, April 23rd, 2026 at 6:00 pm.

Meet and Greets will take place at each elementary school in May. Elementary Schools will contact families directly for appointments (specific dates for each school to be scheduled).
Gainfield Elementary School: May 19 and May 22, 2026
Long Meadow Elementary School: May 18 - May 22, 2026
Middlebury Elementary School: May 27 and May 28, 2026
Pomperaug Elementary School: May 27 and May 28, 2026 -
An Orientation will be held on August 26th, 2026 at each Elementary School.

Voluntary Family Reading History Questionnaire (Provided by the CT State Department of Education)
Families may choose to complete this questionnaire to share information related to language development, early reading behaviors, and any family history of reading or learning challenges. This information helps educators gain a more complete picture of each student and supports instructional planning.
Completion of this form is voluntary and is used alongside other information to support student learning.
