Student Registration
Welcome to Region 15! All school-age students residing in Middlebury and Southbury are entitled to attend public school within Region 15. We are looking forward to getting to know your child and making them feel at home at any one of our highly regarded schools.
Enrollment and Residency Requirements
Prior to registering a child in Region 15, we require proof of residency. Permanent residency (physical presence with an intention to remain) in Southbury and/or Middlebury is the primary requirement to enroll in Region 15 schools. A landowner, taxpayer, or business owner does not confer residency privileges for school enrollment processes.
Registration cannot be completed until all documents have been received and confirmation of Connecticut State Health compliance has been received from the school nurse.
To make the process go smoothly please review the required documentation listed below. All documents must be uploaded during the registration process. Please note you can save your registration and return to the process.
Proof of Residency:
- Current Mortgage Statement, Lease Agreement, or Property Tax Bill
- Current Utility Bill- landline phone, water, electric, gas, oil
- Parent/Guardian Residency Affidavit
For students residing with parents/guardians who are unable to provide a Primary Proof of Residence (Mortgage Statement, Deed, Rental Agreement) - Host Statement of Residency - Affidavit
For families living with someone who is the owner/tenant of a residence in Middlebury or Southbury, CT - Student Residency Affidavit, Parent Permission Statement and Host Statement
For students residing with parents/guardians and living with someone who is the owner/tenant of a residence
Alternative methods for providing residency may be permitted. Please discuss your situation with the Registrar.
The Federal McKinney-Vento Homeless Assistance Act provides special legal protections for children and youth experiencing homelessness, including homeless “unaccompanied youth” who are not in the custody of a parent or guardian. Children experiencing homelessness or displacement have a right to enroll in school even if their families cannot produce enrollment documentation. Assistance is available at the time of registration.
Parent/Guardian Information:
- Parent/Guardian Photo ID: Driver’s License, State Issued Identification or Unexpired Passport
- Custody Agreement (if applicable)
Proof of Age:
- Child’s Birth Certificate or Unexpired Passport
Alternative methods for providing proof of age may be permitted. Please discuss your situation with the Registrar.
Student Academic Records:
- Complete a Student Records Release Form for each student: Student Records Release Form
- Provide a copy of the most recent school transcripts, grades, test scores and credits.
- Provide a copy of your student's IEP or 504 (if applicable).
Student Health Records:
- Entering K-12: Connecticut Health Assessment Record
- Entering PreK: Early Childhood Health Assessment Record
- Required Immunizations
All incoming students must have had an exam within the last 12 months.We can not accept photographs of State Health Assessment Records.

Before and After School Program
The Before and After School (BAS) Program is held at each elementary school in Region 15.
